If you choose to come for therapeutic sessions with me, I will ask you to provide your personal case history which includes name, contact details, date of birth, medical and social history, information about your health and wellbeing. Throughout the course of your sessions, I will keep case notes. All of this information is kept as a paper copy in a locked filing cabinet. I do not store any information in a digital form, except for contact details which are stored in my password protected devices for core business purposes. If you wish to share personal information with me you should arrange for a chat rather than use email.
International Data Transfers
In your use of this website, information which you provide may be transferred to countries which do not have data protection laws equivalent to those in force in the European Economic Area.
I keep case notes to monitor the progress of your sessions. This information is kept strictly confidential and secure. I will keep this information for seven years after the last visit for an adult; or up to the age of 21 plus seven years for a child; or for 15 years rather than 7 in the case of a child or adult who lacks capacity. After this time your information will be destroyed.
I will not share your information without your consent. I will maintain your confidentiality unless I am required by law to disclose your information. I am bound by the professional Code of Ethics of the Craniosacral Therapy Association.
You have the right to know what information is held about you and to correct anything that is not right . Under the GDPR you are not required to give your personal information, but a case history is needed to fulfill our contract, to give you the best treatment and to comply with my Code of Ethics. If you do not wish to give any information I will be unable to treat you.
If you object to me holding your information, you can ask me to stop. However, I will still need to retain your records for a minimum time to comply with my Code of Ethics.
If you request a copy of the information I hold about you I will respond within 28 days.
You have the right to make a complaint if you are unhappy about the way I look after your information or if you feel your rights have not been respected – in the first instance to me and then to my professional body, the CSTA firstname.lastname@example.org or if you are still unhappy to the information Commissioner’s Office (ICO) ico.org.uk/concerns or telephone 0303 1231113.
